Optimizing Systemic Therapy After Prostatectomy

Continuing the prostate cancer session at ESTRO 2026, Dr. Ashwin Sachdeva (The Christie Hospital and the University of Manchester, UK) discussed the role of systemic therapy after radical prostatectomy, focusing on which patients are most likely to benefit from treatment intensification alongside salvage radiotherapy.

A key message was that androgen deprivation therapy (ADT) should not be routinely added for all patients. Current evidence suggests that its greatest benefit is limited to men with high-risk disease, making careful patient selection essential.

The lecture also highlighted emerging strategies beyond conventional ADT. Novel androgen receptor-targeted therapies and other systemic agents are being investigated in the perioperative setting to improve outcomes in patients at increased risk of recurrence, although these approaches remain investigational.
Finally, Dr. Sachdeva emphasized the growing role of genomic profiling and molecular biomarkers in refining risk assessment and guiding future treatment decisions.
